Although we tend to associate it with \u2018oily skin\u2019, hormones or stress, the reality is that <b>acne is a multifactorial condition<\/b>: hormonal, genetic, environmental and inflammatory factors, lifestyle and even gut microbiota all play a role.<\/p>\n<p>In recent years, scientific evidence has shown that <b>diet can directly influence the onset, severity and progression of acne<\/b>. In this article, we explain in simple terms how acne occurs, why certain foods make it worse and what nutritional changes can really help improve your skin.<\/p>\n<p><b>What is acne vulgaris?<\/b><\/p>\n<p>Acne vulgaris is a disease of the pilosebaceous unit. It occurs when the follicle becomes blocked by sebum, keratin and bacteria, causing lesions such as:<\/p>\n<ul>\n<li><b>Closed comedones<\/b> (whiteheads)<\/li>\n<li><b>Open comedones<\/b> (blackheads)<\/li>\n<li><b>Papules<\/b> (inflamed and painful bumps)<\/li>\n<li><b>Pustules<\/b> (with pus)<\/li>\n<li><b>Nodules<\/b> (deep and hard)<\/li>\n<li><b>Cysts<\/b> (inflammatory, painful and with thick content)<\/li>\n<li><b>Scars<\/b><\/li>\n<\/ul>\n<p>It is a disease that occurs in outbreaks and has a strong inflammatory component.<\/p>\n<p><b>How does acne form? The pathophysiology behind the lesions<\/b><\/p>\n<p>Several processes occur simultaneously:<\/p>\n<p><b>1. Sebaceous hypersecretion<\/b><\/p>\n<p>Excess sebum is influenced by androgenic hormones, insulin, and an increase in IGF-1. The latter inhibits the FoxO1 factor and activates the mTORC1 pathway, which stimulates sebum production and cell proliferation in the skin.<\/p>\n<p><b>2. Follicle obstruction<\/b><\/p>\n<p>Hyperproliferation of keratinocytes and their adhesion block the sebum outlet.<\/p>\n<p><b>3. Proliferation of <i>Cutibacterium acnes<\/i><\/b><\/p>\n<p>The lack of oxygen inside the blocked follicle and the abundance of sebum encourage the bacteria to multiply.<\/p>\n<p><b>4. Inflammation<\/b><\/p>\n<p>Oxidised sebum, free fatty acids and the immune response amplify inflammation, causing more painful and persistent lesions.<\/p>\n<p><b>Hormones and acne: the connection we cannot ignore<\/b><\/p>\n<p>The key hormones in acne are:<\/p>\n<ul>\n<li><b>Androgens<\/b>: increase the size of the sebaceous gland and sebum production.<\/li>\n<li><b>Insulin<\/b> and <b>IGF-1<\/b>: enhance cell proliferation, lipid synthesis and the conversion of testosterone to its most active form (DHT).<\/li>\n<li><b>CRH and melanocortins<\/b>: released in stressful situations, they aggravate inflammation.<\/li>\n<\/ul>\n<p>In addition, there are hormonal stages or situations that increase the risk of acne:<\/p>\n<p><b>Polycystic ovary syndrome (PCOS)<\/b><\/p>\n<p>Closely related to insulin resistance and hyperandrogenism.<\/p>\n<p><b>Adult, perimenopausal and menopausal acne<\/b><\/p>\n<p>The drop in oestrogen, insulin resistance and the relative predominance of androgens favour outbreaks.<\/p>\n<p><b>The role of diet in acne<\/b><\/p>\n<p>Diet influences acne because it can:<\/p>\n<ul>\n<li>Increase or reduce inflammation<\/li>\n<li>Alter the gut microbiota<\/li>\n<li>Modify hormone production<\/li>\n<li>Affect insulin and IGF-1 levels<\/li>\n<\/ul>\n<p>Population studies have shown that non-Westernised societies have virtually no acne&#8230; until they adopt a diet rich in ultra-processed foods, dairy products and sugars.<\/p>\n<p><b>Foods and habits that worsen acne<\/b><\/p>\n<p><b>1. High glycaemic index diets<\/b><\/p>\n<p>Foods that rapidly raise blood sugar increase insulin and IGF-1, promoting:<\/p>\n<ul>\n<li>More sebum<\/li>\n<li>More free androgens<\/li>\n<li>More keratinocyte proliferation<\/li>\n<li>Worse inflammation<\/li>\n<\/ul>\n<p>These include: white bread, pastries, sugary cereals, soft drinks, potatoes, white rice, refined pasta.<\/p>\n<p><b>2. Unfermented dairy products (especially skimmed)<\/b><\/p>\n<p>Milk:<\/p>\n<ul>\n<li>Increases insulin (even with a low GI)<\/li>\n<li>Provides IGF-1<\/li>\n<li>Activates mTORC1<\/li>\n<li>Contains proteins (casein and whey) that trigger insulin<\/li>\n<\/ul>\n<p><b>Skimmed milk<\/b> shows a greater link to acne than whole milk.<\/p>\n<p><b>3. Excess saturated and trans fats<\/b><\/p>\n<p>A diet rich in omega-6 and low in omega-3 promotes an inflammatory environment in the skin.<\/p>\n<p><b>4. Milk chocolate or sugar<\/b><\/p>\n<p>The problem is not cocoa, but:<\/p>\n<ul>\n<li>sugar<\/li>\n<li>milk<\/li>\n<li>added fats<\/li>\n<\/ul>\n<p>Dark chocolate with a high percentage of cocoa does not show this effect.<\/p>\n<p><b>5. Excess leucine and glutamine<\/b><\/p>\n<p>Found in whey protein supplements and large amounts of meat \u2192 activate mTORC1.<\/p>\n<p><b>6. Alcohol<\/b><\/p>\n<p>Increases testosterone, worsens the microbiota and can promote bacterial growth on the skin.<\/p>\n<p><b>Foods and nutrients that improve acne<\/b><\/p>\n<p><b>1. Omega-3<\/b><\/p>\n<p>They are anti-inflammatory, reduce IGF-1 and regulate sebum production.<\/p>\n<p>Oily fish, flax seeds, chia seeds, walnuts.<\/p>\n<p><b>2. Essential vitamins<\/b><\/p>\n<p><b>Vitamin A: regenerative and antioxidant<\/b><\/p>\n<p><b>Vitamin C: healing and antioxidant<\/b><\/p>\n<p><b>Vitamin D: low in patients with acne; reduces sebum and C. acnes<\/b><\/p>\n<p><b>Vitamin E: antioxidant and anti-inflammatory<\/b><\/p>\n<p><b>B vitamins: regulate sebaceous secretion (except B12 in excess)<\/b><\/p>\n<p><b>3. Minerals<\/b><\/p>\n<ul>\n<li><b>Zinc:<\/b> bacteriostatic and anti-inflammatory<\/li>\n<li><b>Selenium:<\/b> regulates sebum and reduces oxidative stress<\/li>\n<\/ul>\n<p><b>4. Green tea<\/b><\/p>\n<p>The compound EGCG inhibits mTORC1 and SREBP-1, decreasing sebum production.<\/p>\n<p><b>5. Probiotics<\/b><\/p>\n<p>Restore microbiota, regulate glucose, reduce free androgens and increase SHBG. Lactobacillus acidophilus and L. rhamnosus are particularly useful.<\/p>\n<p><b>6. Antioxidant foods<\/b><\/p>\n<p>The Mediterranean diet is the richest in this group: fruits, vegetables, legumes, EVOO, seeds, nuts, oily fish.<\/p>\n<p><b>Nutritional strategies that help improve acne<\/b><\/p>\n<p><b>1. Low glycaemic index diet<\/b><\/p>\n<p>Reduces:<\/p>\n<ul>\n<li>size of sebaceous glands<\/li>\n<li>sebum production<\/li>\n<li>inflammatory lesions<\/li>\n<li>SREBP-1<\/li>\n<\/ul>\n<p>This is one of the most effective interventions.<\/p>\n<p><b>2. Ketogenic diet<\/b><\/p>\n<p>Improves inflammation and insulin sensitivity. Not necessary for everyone, but may be helpful in some cases.<\/p>\n<p><b>3. Low-meat diet<\/b><\/p>\n<p>Reduces intake of leucine and other amino acids that activate mTORC1.<\/p>\n<p><b>4. Antioxidant-rich diet<\/b><\/p>\n<p>Neutralises free radicals and reduces inflammation, which is key in acne.<\/p>\n<p><b>Conclusion<\/b><\/p>\n<p>Although acne is multifactorial, we now know that diet <b>does influence<\/b> its development and progression.
